The numbered items below are populated using fields on the committee record. The lettered items are populated using fields on the cover page.

1 - Committee ID Number

2 - Committee Name

3 - Committee Mailing Address

4 - Committee Phone Number

5 - Committee Treasurer Name

6 - Committee Record Keeper Name
