35.1. How to Complete the LA State Report Cover Page

 This help documentation will show the data entry required to populate all sections of the LA State Report Cover Page.

The numbered items below are populated using data from the record of the filing committee, the committee officer, the candidate, or ballot measure. The lettered items are populated using fields on the LA State Report criteria page.

NOTE:  Although it does not appear on the PDF report, the FilerID needs to be entered in the Filing Committee's record Registration ID field and LA listed as the state.

 

Criteria Page:

A - Primary Election Date  

B - Beginning of Report Period

C - End of Report Period

D -  Type of Report

 

 

Filing Committee's Record:

1 -  Filing Committee's Name and Address  

 

 

Committee Officer's Record:

2 -  Name of Committee Officer

3 -  Position of Committee Officer

4 -  Address of Committee Officer

Report Preparer's Record: 

5 - Name of  Person Preparing the Report
